St. Padre Pio of Pietrelcina (1887–1968) — already known for his fifty-year stigmata — was also one of the most extensively documented bilocators in the history of the Church. His canonisation process collected sworn testimony from dozens of witnesses who reported seeing him in places he was known not to have been.

The most famous case occurred during the Allied bombing campaign of 1943. American B-17 bomber pilots assigned to attack the German positions at Foggia, twenty miles from San Giovanni Rotondo, reported encountering in the air over the town the figure of a friar — bearded, in brown Capuchin habit, suspended in the sky with his hands raised. Their bombs would not release. Squadron after squadron returned to base without dropping their ordnance. After the war, USAAF general Bernardo Rosini visited San Giovanni Rotondo, recognised Padre Pio as the friar in the sky, and gave his sworn account.

Other documented bilocations: Padre Pio appearing at the deathbed of his confrère in Rome (1905, when he was a young friar in southern Italy); appearing to comfort the dying mother of a soldier; appearing at the canonisation of St. Thérèse of Lisieux in Rome in 1925 (witnessed by multiple Roman clergy); appearing simultaneously in Uruguay to a man who later traced him to San Giovanni Rotondo. Padre Pio when asked about the phenomenon would say, with mild irritation: 'These things happen by the will of God and have nothing to do with me.'
